Birth Certificates

Certified copies of birth certificates are available at the Health Department for births recorded in the State of Maryland from 1939 to present. We cannot access records for any other State besides Maryland.

Who can apply for a birth certificate?

• An individual requesting their own birth certificate
• A parent of the individual - must be named on the certificate
• A court-appointed guardian of the individual - must provide proof
• A representative of the above with a notarized document and valid photo ID

ID requirements:

The applicant must present a valid, unexpired government-issued photo ID.

**Applicants unable to provide valid photo ID- see below**
Must provide two (2) different pieces of mail. The mail must include the correct address on both pieces and be from two different agencies. (The applicant will not receive the birth certificate the same day. The certificate will be mailed to the address displayed on the documents provided).

Payment:
The cost per certificate is $25.00 and acceptable payment methods include cash, Visa, Mastercard, or Discover.
