Destination Wellness
Destination Wellness is a new version of our Highway to Health program.
We focus on helping people who rely on the emergency room (ER) or ambulance (EMS) as their primary healthcare provider.
We can connect you with:- Doctors and specialists
- Help signing up for health insurance
- Support with transportation, utility bills, food, and social services
Healthy Heart Ambassador Program
We help people manage high blood pressure to stay out of the ER and feel better.
We provide:
- A free blood pressure monitor (if needed)
- Easy training on how to use it
- Tips on healthy eating, exercise, and managing stress
- Support from trained Healthy Heart Ambassadors
Why it matters:
- Helps your doctor understand your blood pressure over time
- Catches problems early
- Lowers your risk of heart attacks, strokes, and hospital visits
- Helps you stay in control of your health
Peer Recovery Programs
We support people dealing with substance use. No judgment, just help.
Harm Reduction Services
- Clean and safe use and substance testing
- Narcan to prevent overdoses
- Testing for HIV, hepatitis C, and syphilis
- Links to treatment and mental health care

Health Equity Peer Program
For people without stable housing or leaving the hospital:
- One-on-one peer support
- Help with housing, ID, and paperwork
- Narcan training and connection to support groups
- Referrals to doctors and treatment programs

Community Outreach & Social Support
This is the heart of everything we do.
Our Community Health Workers meet people where they are and help with all things that affect your health- like housing, food, transportation, and safety.
They work to:
- Understand what’s making it hard for you to stay healthy
- Connect you with local help (food banks, community organizations, support, and more)
- Teach you how to take control of your health
- Work with local groups to build a healthier, stronger Calvert County
It’s about fixing the root problems- not just the symptoms. Our goal is simple, that everyone has a fair chance at better health.
