Certified Copies of Death Certificates

Certified copies of death certificates are available at the Health Department for death records in the State of Maryland from 2015 to present.

*For records BEFORE 2012, contact the Maryland State archives at 410-260-6400*.

Who can apply?

• The informant listed on the death certificate
• A parent listed on the death certificate
• A surviving relative - must provide proof
• A beneficiary - must provide proof
• A creditor of the deceased
• A person who submits a court order that commands a copy be produced
• Funeral director

ID Requirements:
The applicant must present a valid, unexpired government-issued photo ID.

Documentation Requirements

If not listed on the death certificate, you are required to provide proof of relation by presenting one of the following:

• Birth certificate
• Obituary- must list your name
  
• Letter of administration
  
• Authorization of release
  
• Court order
  
• Letter from insurance company
  
• Deed
  
• Vehicle title

Payment:

The cost per certificate is $25.00 and acceptable payment methods include cash, Visa, Mastercard, or Discover.
